Lost or Stolen Proxy Cards
If your KU Library Proxy Card has been lost or stolen, the proxy or the proxy sponsor must deactivate it immediately in order to prevent unauthorized use and charges.
Step 1: Contact the Circulation Business Office in Watson Library in person at the Watson Library Service Desk, 8:00 a.m.-5:00 p.m., Monday-Friday; by phone to 785.864.4715, or via email to libfines@ku.edu.
- The Circulation Business Office will determine if the card has been found or that it needs to be declared lost.
- You are responsible for any items checked out and for any charges incurred with the use of your KU Library Proxy Card. Contacting the Circulation Business Office is an essential step.
Step 2: To retain library privileges, request a new Proxy Card. The proxy sponsor will need to complete a new Proxy Card Application Form. See Proxy Cards for information.
Individuals who acquired their KU Library Proxy Cards at the Edwards Campus may report lost or stolen card and apply for a replacement card at the Regents Center Library (913.897.8570).
